A Bulgarian ICT company has developed a revolutionary solution in the field of personalized orthopedics. The solution is a mixed reality application based on Microsoft Hololens 2.
In the world of orthopedic surgeries, around 20% of the patients continue feeling pain and discomfort after the intervention and often they require additional surgeries. Surgeons do their best to execute the surgery according to their plan but sometimes they can make mistakes in the different stages of the operation. Currently, in order to improve surgical precision, the main solutions used on the market are surgical guides, that have the disadvantage of being expensive and often come with logistics issues such as the need to be tailor-made for the surgery.
The software by the Bulgarian company allows viewing and interacting with 3D holograms of the patient’s bones in an entirely new way. The solution superimposes the 3D hologram of the patient’s bone with a great precision. With the use of fiducial markers, the software tracks the position of the surgeon’s tool in real time relative to the preoperative plan made by the surgeon. Furthermore, the whole surgery is recorded and uploaded in Microsoft Azure with all consisting elements and actions. Recordings can serve as great teaching and education material as well as evidences in court cases. The software is available on the Microsoft Store and can be downloaded on any Hololens 2 device.
The company is looking for medical universities with orthopedic specialties to help validate the technology and conduct clinical tests and studies. - Advantages and Innovations

While developing the solution, the company has achieved several key technological milestones. As standard, Hololens 2 can track one marker at a time that is 15x15cm in size. The solution offered can track up to 10 markers simultaneously with a marker size of 3.5x3.5cm without sacrificing accuracy. Furthermore, the standard refresh rate of the headset is once per second, which would not be suitable for surgical procedures. With company’s proprietary algorithms the solution has a refresh rate that is 50 times faster than standard – once per 20 milliseconds.
Mixed reality is a game changing technology for healthcare as it aids surgeons to be better at actions with high precision. A “supervision” for surgeons can be created that displays all relevant information at a glance instead of observing monitors all around the Operating Room.
Mixed reality is also an amazing tool for teaching and education, where simulations of all sorts of patient scenarios can be made by providing haptic feedback from the instruments. - Stage of Development

The team is already partnering with a medical university from The Netherlands that implements planned surgeries on cadavers and animals with the technology offered.
